Application Instructions:

1.) Click on the PDF button. This will open a new browser tab. You can fill out the information via your browser or you can download the file to your computer and fill it out that way.

2.) Once the application is filled out, save the file to your computer.

3.) Email the filled out application to fitchburgculturalalliance@gmail.com with the subject "MUSIC FOR YOUTH APPLICATION".
